In order to receive this free package you must first complete, sign and submit both the (white) dues deduction form and (blue) COPE deduction form. They can be downloaded at www.ntuaft.com/Info_Center/Forms/forms.html. Once we have verified receipt of your forms, we will send you an email letting you know your package is ready to be picked up at the NTU office.
